How the Process Works:

A standard bariatric evaluation is efficient, transparent, and broken into three straightforward steps:

1. Upfront Digital Paperwork (approximately 2 hours total): From home, you’ll complete secure online questionnaires about your health, lifestyle, and mental health. You may take breaks between forms or complete them over several days. Please submit all forms at least 48 hours before your appointment so I have time to review them.

2. Virtual Clinical Interview (50–60 minutes): We will meet for a collaborative telehealth video call to discuss your history, your preparation for the post-surgery lifestyle, your support systems, and look in depth at anything that arises from your upfront paperwork.

3. Report Submission: I compile the data into a comprehensive, professional report and transmit it directly to your bariatric surgeon’s office.

Cost & Insurance

I am in network with Aetna and Horizon Blue Cross Blue Shield. Your cost depends on your plan’s deductible, copay, or coinsurance.

Self-pay rate: $550. This flat fee covers the questionnaires, clinical interview, interpretation, and written report. Payment is due when you book your appointment.
